Hope you are well!

I have been notified that your company will be running an extra-curricular club next term at Whitehouse and will sending in a Coach to teach the children (or possibly it is yourself will be coming in to school).

I need to make sure we have all checks in place (DBS, letter of assurance from your company etc) for this academic year and before clubs begin.

Please could you confirm the name of the coach that will be coming?

I will then need the following from you please:

1. A company letter of assurance (see document attached – you can just fill this in electronically and return rather than having to write a letter yourself) This should include a full list of coaches coming with their DBS number and DBS issue date.
   

1. Completed childcare disqualification form (attached) to be signed by the coach coming in.
   
2. Signed processes and procedures induction form (attached) to be signed by the coach coming in.
   
3. First aid certificate for the coach coming in.
   
4. Safeguarding certificate for the coach confirming they have had safeguarding children and young people training
   
5. Your public liability insurance certificate
   
6. Contact details (phone number, address and date of birth) of the coach you are sending (this so they can be added to our School Central Record)
   

On the first day of their visit the coach will also need to bring their original DBS certificate and a photo ID.
